Unsafe Hospital Discharges

Healthwatch Telford and Wrekin have heard from different care homes of times when discharges to them from hospital have been unsafe in their opinions. Likewise, we have heard from people who have mixed experiences of discharge. This is becoming more prevalent. We aim to work with care homes to explore this is in more detail with a survey and focus groups for care homes representative to jointly identify areas of stress within the system that leads to unsafe discharges and the impact of these. We will also promote this to the public to seek individual experiences of unsafe discharge and seek any similarities between the two to combine into one report with recommendations for improvement.

If you or a loved one have been discharged from hospital to a care home or back to your or their own home let us know how it was for you? Was it all smooth and everything you needed in place? You knew what was happening and it all went as planned? Didn't it go to plan? If you were discharge to a care home did you have to be readmitted to hospital straight away? Was no support in place at home? When you were first admitted to hospital were you involved in planning for when you would be discharged? 

Your experience could be any one or two or more of these experiences or it could be a different scenario. The key thing it is YOUR experience - share it with us so we can create a report based on your truths and experiences and look for where things could be improved or suggest how things that work well could be done similarly in other areas.
